Duties
- Assist clients with activities of daily living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods such as Hoyer lifts.
- Monitor patient health by taking vital signs, observing for changes in condition, and reporting findings accurately while adhering to HIPAA regulations to protect client confidentiality.
- Administer medications and assist with medication management according to care plans and physician instructions.
- Prepare nutritious meals and assist with meal planning, ensuring dietary needs are met in accordance with individual care plans.
- Support clients during community outings or appointments by providing personal transportation assistance and ensuring safety throughout activities.
- Maintain a clean and safe environment by performing routine cleaning tasks, sanitizing equipment, and adhering to infection control protocols.
- Document client progress, update care plans, and communicate effectively with team members regarding patient status or any concerns related to dementia care, autism support, or neurodevelopmental disorders.
Skills
- Proven experience working in group homes, assisted living facilities, nursing homes, or home health settings.
- Strong knowledge of patient observation techniques, vital signs monitoring, and medication administration.
- Familiarity with Alzheimer's care, memory care strategies, dementia support, and hospice or palliative care practices.
- Ability to perform patient transfers safely using Hoyer lifts or other manual handling equipment; experience with patient transfer methods is essential.
- Skilled in assisting with ADLs such as bathing, dressing, grooming, meal preparation, and personal care assistance.
- Knowledge of HIPAA compliance standards to ensure confidentiality of client information at all times.
- Experience working with individuals with developmental disabilities or neurodevelopmental disorders like autism is highly desirable.
- Capable of heavy lifting and physical tasks required for patient transfer or mobility assistance; CPR and first aid certifications are preferred.
- Excellent communication skills for engaging effectively with clients, families, and team members; social work background is a plus.
- Ability to follow detailed care plans while demonstrating patience and compassion in all interactions.
