- Location: Greeley, Loveland, Fort Collins and surrounding areas.

Visit Rates: $75-$150, (depending on visit type), PLUS mileage and travel time compensation!

Home Health Physical Therapist
We are seeking an energetic, self-motivated, hardworking, and dependable Physical Therapist (PT) to provide exceptional care to clients in the comfort of their own homes. The ideal candidate is organized, adaptable, and able to multitask effectively while managing a dynamic caseload.
The successful candidate will be comfortable working independently, demonstrate strong clinical and communication skills, and be committed to delivering high-quality, compassionate patient care. Experience with OASIS documentation is preferred; however, we are happy to provide training for candidates who are willing to learn.

Responsibilities
-
Complete initial and ongoing comprehensive assessments as assigned.

Provide interventions and therapeutic treatment

Effectively communicate with other members of the interdisciplinary healthcare team to promote the coordination of patient care and planning for discharge.

Participate in Quality Assessment and Performance Improvement (QAPI) program.

Responsible for reporting on the condition of patient to the Rehab Director, and other appropriate staff members.
Required Qualifications

Current Physical Therapist (PT) licensure in the state of Colorado

Current Basic Life Support (BLS) CPR or willing to obtain upon hire (demonstration course required)

One year of PT experience

Home health care experience preferred, but not required

Pass a background check and drug test (we do to test for THC)
