This Family Model Provider service could be considered similar to what was formerly known as "Adult Foster Care".
Progress Inc. is a provider agency that has served adults with intellectual disabilities and seniors in Davidson and surrounding counties since 1971. We provide day, residential and employment support services to individuals we serve through DDA (TN Dept. of Disability and Aging), ECF Choices and Choices (Insurance Companies / TN Care) funded programs.
- We are seeking to contract with providers living in the Nashville or Franklin area that own or rent their home and have the time and extra space to support senior(s) or adult(s) with or without a disability needing a place to live and receive supports.
- Compensation is a daily rate based on the level of subsequent supervision / care that the individual(s) require. The provider will also receive a monthly stipend for room and board.

General Duties:
- Take responsibility for overseeing the individual's care and health, aspects of daily living.
- Consider the individual being supported in your home as an equal member of the home. No Exceptions
- Allow individual(s) free use of common living areas and amenities in the home
- Notify Progress management of any significant event effecting the client's well-being or provider's ability to provide services
- Maintain and submit all required records for individuals to Progress management
- Participate in the development and implementation of the individual's plan of service
Requirements:
- Home must be inspected and meet requirements as a Family Model Provider using a site survey tool
- Residence MUST have an extra private bedroom for individual supported and at least a shared bathroom
- Anyone having contact with the individuals served must not be listed on any registries reporting any form of fraud or abuse.
- Any other adults living in the home must pass a criminal background check.
- Provider must have a valid DL, insurance (vehicle and homeowners / renter's) and reliable transportation.
- Provider and any staff paid to assist must complete approximately 40 hours of mandatory training.
- Must have a high school diploma.
Job Type: Contract
Pay: $100.00 - $175.00 per day
