Position: HCBS Coordinator / Spanish Speaker
Schedule: Monday- Friday 7:30 AM-4:00 PM

Core Responsibilities:
- Answering and returning calls to new potential providers and/ or members in a timely manner.

Providing accurate and timely information to potential providers/members/vendors, etc. in a professional and courteous manner.

Assist with identifying areas of improvement and implementing with the Spanish Speaking families with the onboarding process of potential providers and members.

May participate/ run potential provider interviews.
- Assist with day-to-day operations of the HCBS department regarding all providers and members.

Assist HCBS providers with:

Vtrack
-
Scheduling/ Calendars
- 90-Day ATC Monitoring Meetings

Assist with onboarding for our Spanish speaking families
-
Matching & Interviewing Spanish speaking providers with families
-
Participating in DDD (Division of Developmental Disabilities) required meetings, such as but not limited to:
- DDD Planning Documents
-
Review and enter goals into Vtrack
- Communicate with parents/guardians and providers regularly to ensure service goals are met.

Monitor and maintain agency compliance with requirements set forth by the DDD (Division of Developmental Disabilities) for Qualified vendor agencies.

Comply with the implementation of all operation strategies and tactics that are consistent with Tyler's Place Az mission, vision, and values.

Regularly communicate and work closely with the agency Directors and Executives.

Keep accurate notes via spreadsheets and/or VTrack as required.

Keep accurate and up to date documentation; paper or electronic as required.

Strong customer service and communication skills with members, families, vendors, and staff.

Willingness to be coached and trained.

Strong attention to detail, organizational skills, and the ability to multi-task to meet deadlines.

Excellent communication skills and the ability to manage all relationships.

Must be reliable and maintain a positive attitude and compassionate approach.

Be able to thrive in a professional office setting where you must work alongside others.

Be able to answer phone calls, texts, and emails throughout the day, maybe after hours as needed.

Participate in troubleshooting issues that may arise for parents/ guardians, providers, and members as needed.

Identify issues and address those needs in a timely manner.

Be available to attend the occasional after-hours events hosted by Tyler's Place.

Additional duties may be assigned, and existing duties may be modified or removed at the discretion of the organization.

Required Qualifications:
- Spanish Speaker

The ability to obtain a State of Az Level 1 Fingerprint Clearance Card and pass a nationwide background check.

Must possess a valid Az drivers license.

Keep up to date with on-going Professional Development Trainings such as: Article 9, First Aid/ CPR, CEUs and DCW.

High School diploma required; advanced education or experience preferred.
