Job Summary
We are seeking a compassionate and dedicated Caregiver/Home Health Aide to provide exceptional support and assistance to individuals in their homes or community settings. This vital role involves delivering personalized care, promoting independence, and ensuring the safety and well-being of clients across various care environments. Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and document patients’ vital signs, health status, and behavioral changes to inform care plans
- Provide medication reminders and assist with medication administration in accordance with HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
- Support patients with Alzheimer’s, dementia, or memory care needs through specialized techniques and compassionate engagement
- Operate assistive devices such as Hoyer lifts and ensure proper handling during transfers to prevent injury
- Maintain a clean, safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing personal spaces
- Observe patient conditions closely, report concerns promptly, and collaborate with healthcare professionals to ensure optimal care
- Support residents in group home or assisted living settings by fostering a respectful, engaging atmosphere
- Follow care plans meticulously while respecting client privacy and confidentiality under HIPAA guidelines
Qualifications
- Prior experience in home health care, assisted living, nursing homes, or group homes is highly preferred
- Certification in CPR and First Aid is required; additional certifications such as Alzheimer's or dementia care are a plus
- Knowledge of HIPAA regulations and patient privacy standards necessary for compliance
- Ability to work with individuals with developmental disabilities, autism spectrum disorder, or other special needs
- Strong observational skills for patient monitoring and reporting changes in condition
- Physical ability to perform heavy lifting and assist with mobility support using equipment like Hoyer lifts
- Excellent communication skills; compassionate attitude; reliable team player committed to quality care
- Experience working with diverse populations including seniors, individuals with disabilities or chronic illnesses such as hospice or memory care patients
