POSITION: Case Manager
RESPONSIBLE TO: Director of Client Services —( DCS)
SUPERVISION EXERCISED: None
JOB SUMMARY: Must be highly motivated, organized with strong work ethics.
Case Managers (CM) work under the direct supervision of the Director of Client Services (DCS) and provide services to individuals in Garland, Hot Spring, Clark, Montgomery and Pike counties. The CM carries a case load for Targeted Case Management and is responsible for locating, coordinating and monitoring necessary and appropriate services for a client. The CM assesses and assists clients in gaining access to all needed medical and social services and also works with individuals who are at risk of abuse, neglect or exploitation in order to determine the proper placement and/or need for services. The CM is responsible for examining and re-examining the client's needs and alters the service plan to meet the changing needs. The CM provides Medicare counseling during open enrollment.
DUTIES & RESPONSIBILITIES:
- Conduct initial assessment of referrals in a professional and timely manner.
- Gather and evaluate intake information and assess problems and needs and determine the appropriate type of assistance needed. Assist with completing paperwork.
- Conduct outreach, provide information on and determine eligibility for public benefits such as QMB and SMB that the client has not yet received.
- Assist client in completion of applications and paperwork.
- Attend meetings on behalf of client and provide information and assistance when needed.
- Develop thorough initial service plan that addresses the needs on all clients age 60 and older.
- Contact service providers and negotiate for the delivery of services identified in the service plan in a professional and timely manner.
- Verify through regular contacts with service providers and client that appropriate services are provided in accordance with the service plan and that the recipient is satisfied with services.
- Develop and maintain sufficient written documentation to support each service for which billing is made.
- Update service plan as changes occur but at least every 6 months by reexamining the clients problems and needs so that changes that have occurred since the previous assessment can be addressed.
l l . Update intake on targeted case management (TCM) clients on a yearly basis.
- Maintain a current list of all assigned TCM clients in the computer, updating information as changes occur.
- Maintain good working relationship with Senior Centers in assigned area visiting at least once a month.
- Maintain good working relationship with all service providers in assigned area.
- Advocate for the rights of the elderly on an individual or group basis as needed.
- Represent the agency at training sessions, public hearings and meetings as assigned.
- Disseminate information in regard to services, programs, legislation and other areas of interest to the elderly.
- Inform DCS of any significant problems found with clients and service providers.
- Actively participate in developing departmental and program goals.
- Actively participate in office meetings reporting status of clients and services provided.
- Actively participate in public relation and marketing activities.
- Assist with office duties as requested.
- Participate in quarterly Quality Assurance and Record Review meetings.
- Other duties as assigned.
COMPETENCY REQUIREMENTS:
Knowledge of local community resources in regard to the elderly population.
Knowledge of services available through SSI/Social Security, Medicare, Medicaid, V.A., Legal
Services, Services for the Blind and other sources of assistance for the elderly.
Knowledge of problems and needs of the elderly.
Ability to effectively communicate verbally and in writing with others.
Ability to work independently and be self-motivated.
Possess good computer skills, including the use of Microsoft Word, Excel, Outlook and how to access information on the internet.
Must be able to drive and have dependable transportation with current auto liability insurance and driver's license. Travel is required.

MINIMUM REQUIRED QUALIFICATIONS:
Bachelor's degree in social work, gerontology, sociology or related field or LPN.
