Position Summary
TEAMS LLC is seeking an experienced and dependable Community Support Team Lead to provide clinical leadership and direct community-based services to adults with serious mental illness and complex behavioral health needs. The Team Lead will oversee service delivery, coordinate care, support CST staff, and ensure services are provided in accordance with each member’s Person-Centered Plan and applicable Medicaid requirements.
Primary Responsibilities
- Provide clinical leadership and oversight for the Community Support Team.
- Deliver direct CST services in members’ homes and community settings.
- Complete assessments, Person-Centered Plans, crisis plans, service notes, and authorization-related documentation.
- Coordinate services with medical providers, behavioral health professionals, hospitals, housing agencies, and community resources.
- Monitor member progress, safety concerns, treatment engagement, and service effectiveness.
- Provide supervision and direction to CST team members.
- Participate in team meetings, discharge planning, crisis response, and care coordination.
- Ensure documentation is accurate, timely, and compliant with Medicaid and agency standards.
- Travel throughout Gaston, Cabarrus, Rowan, and Union Counties as needed.
Minimum Qualifications
- Must meet current North Carolina qualifications for a Community Support Team Lead.
- Must be a fully licensed behavioral health professional in North Carolina.
- Experience working with adults with serious mental illness is required.
- Valid driver’s license, reliable transportation, and current automobile insurance are required.
- Must be able to work independently, respond appropriately to crises, and manage community-based responsibilities.
- Strong documentation, leadership, communication, and care-coordination skills are required.

Position Details
This is a contract position with a flexible community-based schedule. Compensation will be discussed based on experience, qualifications, availability, and assigned caseload.

Pay: $60.00 - $75.00 per hour
