Planning and managing patient care according to each patient's needs. Interviewing patients and recording their medical history. Obtaining patient vital signs and escorting patients to their rooms.
Essential Duties and Responsibilities: Include the following. Other duties may be assigned.
1. Chart abstraction. Preview last office note/encounter to obtain any previous tests, lab reports or any pertinent medical information prior to new patient appointment.
2. Answers phone, initiates telephone encounters.
3. Schedules appointments. Confers with physician for add on appointments.
4. Completes medication refill requests through EPIC.
5. Return calls for doctors at their direction regarding tests results, labs, medication questions, etc.
6. Obtains prior authorizations and coordinates patient’s enrollment for pharmacy assistance programs when necessary.
7. Completes FMLA, Disability forms, return to work letters, etc.
8. Performs other administrative duties as needed as back up to coworkers in fulfilling their duties in their absence or during a heavy work period as requested by management.
Employee is required to sit and walk frequently. Employee must lift/carry a light (1-20 lbs.) load frequently, a medium (21-50 lbs.) and/or heavy (50 lbs. +) load occasionally. Employee may be required to lift over 100 lbs. with assistance. Employee is occasionally required to bend, squat, crawl, kneel, climb, lift waist to overhead. Employee is occasionally required to reach forward and reach overhead. Employee may be required to do simple grasping and pushing/pulling. (Occasionally: 1-33%, Frequently: 34-66%, Continuously: 67-100%) The most significant of duties are included but this does not exclude occasional work assignments not mentioned or developmental duties.

High school diploma required. Graduate of LPN program with current licensure required.
