Case Manager – SOURCE / EDWP Program
Position Summary
We are seeking a compassionate, organized, and dependable Case Manager to support members enrolled in the SOURCE / Elderly and Disabled Waiver Program (EDWP). The Case Manager will help identify appropriate providers, facilities, services, and community resources across the continuum of care while ensuring services are delivered in a timely, compliant, and cost-effective manner for both the member and the reimbursement source.
This role requires strong communication, documentation, time management, and problem-solving skills, along with a commitment to promoting quality care, member independence, and positive health outcomes.
Key Responsibilities
- Maintain working knowledge of applicable SOURCE / EDWP policies, procedures, and state program requirements.
- Assess, plan, coordinate, monitor, and evaluate services to meet each member’s health, safety, functional, and service needs.
- Identify appropriate providers, facilities, and community-based resources to support member care and independence.
- Encourage skill development in Activities of Daily Living through appropriate task completion, coaching, and support.
- Promote quality, cost-effective interventions that improve member outcomes and support efficient use of available resources.
- Develop and maintain respectful, professional relationships with members, responsible parties, families, providers, and interdisciplinary team members.
- Communicate clearly and consistently with members, families, and care teams to promote coordinated, high-quality services.
- Support member self-management skills, including problem solving, decision making, communication, conflict resolution, anger management, and social or recreational skill development.
- Implement member goals and care plans focused on improving health, safety, independence, and overall well-being.
- Partner with community organizations and service providers to connect members with opportunities and supports that improve quality of life.
- Accurately complete case notes, progress notes, event documentation, reports, and other required records in accordance with company policy and program requirements.
- Maintain professional boundaries while providing compassionate, member-centered support.
Required Qualifications
- Bachelor’s degree required.
- Valid driver’s license.
- Reliable transportation.
- Basic computer skills, including Microsoft Outlook and Microsoft Word.
- Access to a laptop for documentation and communication.
- Ability to prepare, maintain, and submit accurate reports and records.
- Ability to work independently, manage time effectively, and prioritize competing responsibilities.
- Strong professional communication, interpersonal, and information-sharing skills.
- Willingness and ability to resolve complex and sensitive issues professionally.
- Ability to maintain a positive working relationship with a wide variety of individuals, families, providers, organizations, and community partners.
Knowledge, Skills, and Abilities
- Knowledge of case management principles, service coordination, and member-centered care planning.
- Understanding of applicable laws, rules, regulations, policies, and procedures related to community-based services.
- Familiarity with screening, assessment, monitoring, and documentation practices.
- Ability to identify and connect members with appropriate services, supports, and community resources.
- Proficiency with Microsoft Outlook and Microsoft Word.
- Ability to communicate effectively with members, responsible parties, families, providers, and internal team members.
- Ability to remain calm, professional, and solution-focused in stressful or sensitive situations.
Preferred Qualifications
- Prior experience in case management, care coordination, home health, community health, aging services, disability services, or Medicaid waiver programs preferred.
- Experience working with elderly individuals, individuals with disabilities, families, caregivers, and community-based service providers is a plus.
- Training will be provided for qualified candidates who demonstrate strong professionalism, reliability, and a willingness to learn.
