We are seeking a Full-Time Physical Therapist (PT) to support our skilled nursing therapy team on an as-needed basis. These roles are ideal for licensed clinicians looking for flexible scheduling while delivering high-quality rehabilitation services in a skilled nursing environment.
Key Responsibilities
PT Responsibilities:
- Perform evaluations and develop individualized physical therapy treatment plans
- Supervise PTA treatment when applicable
- Collaborate with interdisciplinary teams to support resident goals and discharge planning
- Provide skilled therapy services to short-term rehab and long-term care residents
- Complete accurate and timely documentation in accordance with Medicare and facility guidelines
- Participate in care planning and interdisciplinary communication as needed
Qualifications
- Active PT in WI.
- Graduate of an accredited Physical Therapy program
- SNF or post-acute care experience preferred (new graduates welcome)
- Strong communication, documentation, and time-management skills

- Weekdays and/or weekends based on facility needs
