Housing Case Manager
Position Title: Foundational Community Supports (FCS) Housing Case Manager

Position Summary
The Foundational Community Supports (FCS) Housing Case Manager provides person-centered, community-based services to help eligible individuals obtain and maintain stable housing under the Washington State Foundational Community Supports program, administered by Wellpoint and overseen by the Washington State Health Care Authority (HCA). This position supports individuals experiencing homelessness, housing instability, or institutionalization risk through engagement, assessment, housing navigation, tenancy support, and care coordination. The FCS Housing Case Manager ensures all services are delivered in accordance with fidelity to FCS program standards, emphasizing Housing First, trauma-informed care, harm reduction, and person-centered recovery principles.
Essential Duties & Responsibilities
- Conduct outreach and engagement to identify and enroll eligible participants into the FCS Housing program.
- Complete enrollment and authorization documentation accurately and within required timelines.
- Conduct comprehensive housing assessments and develop individualized service plans.
- Provide ongoing case management, housing navigation, and tenancy support services.
- Assist participants with securing identification, income benefits, and supportive resources.
- Support participants with landlord communication and move-in coordination.
- Educate participants on tenant rights, lease adherence, and housing maintenance responsibilities.
- Coordinate with behavioral health, substance use, and physical health providers.
- Maintain relationships with landlords and housing providers to expand housing opportunities.
- Document all services and outcomes in compliance with FCS reporting standards.
Performance Metrics & Fidelity Standards
- Maintain caseload of 20–25 participants per full-time Housing Case Manager.
- Provide community-based contacts per fidelity requirements (minimum one per month when housed).
- Ensure timely documentation of services and outcomes per Wellpoint and HCA standards.
- Achieve housing stability benchmarks (90+ and 180+ days) and fidelity compliance scores.
Required Qualifications
- Bachelor’s degree in Social Work, Human Services, or related field; equivalent experience considered.
- Minimum 2 years of housing case management or supportive services experience.
- Knowledge of Housing First, trauma-informed care, and harm reduction principles.
- Proficiency in documentation, communication, and data systems.
Preferred Qualifications
- FCS Housing Specialist certification or direct FCS experience.
- Experience working with Medicaid-eligible populations or behavioral health needs.
- Knowledge of Spokane-area housing systems and resources.
Work Environment / Conditions
- Primarily field-based with regular travel in Spokane County.
- Occasional office work, meetings, and evening/weekend hours as needed.
- Must maintain valid driver’s license, reliable transportation, and insurance.
