Position Summary:
We are seeking a dedicated Part-Time Nurse to oversee client care plans and provide support to both caregivers and veterans receiving services through our home care programs. This position is ideal for a nurse who enjoys relationship-based care, flexible scheduling, and meaningful work supporting those who have served our country.
Nurse supervisory visits to clients occur once monthly and when the caregiver is present. Most client shifts are during the morning and afternoon hours. Nurse scheduling is completely flexible, but must be during a time when the caregiver will be with the client.
Key Responsibilities:
- Review, update, and manage individualized care plans.
- Conduct client assessments and periodic supervisory visits as required.
- Provide guidance and support to caregivers to ensure high-quality care delivery.
- Communicate regularly with clients, families, and the office team to coordinate care.
- Maintain compliance with all agency policies and state regulations.
Qualifications:
- Active Texas RN license.
- Prior home health or home care experience preferred.
- Excellent communication and documentation skills.
- Compassionate, dependable, and committed to veteran-centered care.
- Reliable transportation and ability to travel locally as needed.
