Responsibilities
- Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote independence and dignity.
- Monitor and record vital signs including blood pressure, pulse, temperature, and respiration rates accurately in electronic health records (EHR) systems.
- Support medical procedures by preparing patients, maintaining aseptic technique, and assisting with tasks like catheter care or phlebotomy when trained.
- Ensure patient safety through safe manual handling practices, proper patient transfer methods using equipment like Hoyer lifts, and adherence to infection control protocols.
- Communicate effectively with patients, families, and healthcare team members to relay important information about patient status or needs.
- Maintain strict confidentiality of patient information in compliance with HIPAA regulations and hospital policies.
- Document all care activities thoroughly in medical documentation systems such as EMR/EHR platforms to ensure accurate medical records.
Skills
- Proven experience working with diverse populations including geriatric patients, individuals with developmental disabilities, dementia or Alzheimer’s disease, behavioral health needs, or those requiring hospice or palliative care.
- Familiarity with EMR systems like Cerner or eClinicalWorks for efficient documentation and patient record management.
- Knowledge of medical terminology, anatomy, physiology, and basic life support (BLS) techniques including CPR.
- Ability to perform heavy lifting safely and execute safe lifting practices during patient transfers or repositioning.
- Experience working in various healthcare settings such as long-term care facilities, hospitals (including ICU and trauma centers), outpatient clinics, or home & community care environments.
- Skilled in using assistive devices like Hoyer lifts and understanding patient transfer methods to ensure safety for both patients and caregivers.
- Strong caregiver communication skills combined with a compassionate approach to patient service delivery.
- Ability to work effectively under pressure in fast-paced environments such as emergency medicine or urgent care settings.
- Knowledge of infection control procedures including aseptic technique and infectious disease care protocols.
