Job Summary: The Home Care Coordinator is responsible for coordinating and overseeing the delivery of home care services for Cherry Blossom PACE participants. This position collaborates with the Interdisciplinary Team (IDT) to assess participant needs, develop and implement individualized care plans, supervise home care staff, and ensure services are delivered safely, effectively, and in compliance with PACE regulations and organizational policies.
The Home Care Coordinator promotes participant independence and quality of life while ensuring participants receive high-quality care that enables them to remain safely in their homes whenever possible.
Education, Licenses, Certifications and Experience:

High school diploma or GED required.
- Current Virginia Certified Nursing Assistant (CNA) license in good standing.
- Current CPR/BLS certification.
- Minimum two (2) years of experience providing direct care to frail or elderly adults in a home care, PACE, long-term care, assisted living, or similar healthcare setting.
- Previous experience serving as a Lead CNA, Team Lead, Home Care Coordinator, Scheduler, or other healthcare leadership role.
- Demonstrated ability to coordinate staff schedules, prioritize participant needs, and communicate effectively with interdisciplinary healthcare teams.
- Valid driver's license, reliable transportation, and acceptable driving record.
Preferred

Previous PACE experience.
- Supervisory or scheduling experience in home care or long-term care.
- Experience using electronic medical records (EMR).
- Bilingual skills are a plus.
