Job Summary:
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Beavercreek Health and Rehab, a premier care community in Beavercreek, Ohio. Responsibilities:
Provide direct patient care to residents, including administering medications, wound care, and other treatments as prescribed by physicians
Conduct routine vital sign checks and document patient information accurately
Develop and implement individualized care plans to meet the unique needs of each resident
Collaborate with other healthcare professionals, including physicians, physical therapists, and occupational therapists, to ensure comprehensive care
Communicate effectively with residents, families, and staff to ensure open and transparent care
Maintain accurate and up-to-date medical records and reports
Participate in quality improvement initiatives and maintain high standards of care
Support and promote a positive and engaging environment for residents and staff
