Job Title: Social Worker – THPP-NMD Foster Care Program (Ages18-21)

Job Summary:
The Social Worker for the Transitional Housing Placement Program for Non-Minor
Dependents (THPP-NMD) is responsible for providing case management, advocacy, and
supportive services to foster youth ages 18-21 as they transition to independent living. The
Social Worker ensures that program participants receive the necessary resources, guidance, and emotional support to develop life skills, secure stable housing, and achieve self-sufficiency.
Key Responsibilities:
Case Management & Support Services
- Maintain a caseload of assigned youth and provide individualized case management.
- Develop and implement Transitional Independent Living Plans (TILP) tailored to each youth’s goals and needs.
- Conduct regular home visits, in-person check-ins, and assessments to monitor progress.
- Provide crisis intervention and conflict resolution as needed.
- Assist youth with obtaining and maintaining stable housing, employment, and education.
- Connect participants with appropriate community resources, including mental health services, vocational programs, and life skills training.
Compliance & Documentation
- Ensure that all case notes, assessments, and reports are completed accurately and on time.
- Maintain compliance with county, state, and federal regulations, including California
Department of Social Services (CDSS) and Community Care Licensing (CCL) requirements.
- Participate in audits, quality assurance reviews, and program evaluations.
- Ensure all documentation related to youth progress, incidents, and interventions is properly recorded.
Advocacy & Collaboration
- Advocate for youth rights and needs within the foster care and social services systems.
- Collaborate with county social workers, mental health professionals, probation officers, and other stakeholders.
- Attend multidisciplinary team meetings, court hearings, and case conferences as needed.
- Facilitate workshops and training sessions for youth on life skills, financial literacy, and housing stability.
Program Development & Community Engagement
- Assist in enhancing program services based on the evolving needs of youth.
- Build and maintain relationships with landlords, employers, and community organizations to create more opportunities for participants.
- Support youth in developing self-sufficiency skills, including budgeting, job searching, and time management.
Qualifications & Requirements:
- Education: Bachelor’s degree in Social Work (BSW) or related field required; Master’s degree in Social Work (MSW) preferred.
- Experience: Minimum of 2 years of experience in child welfare, foster care, transitional housing, or a related field.
- Knowledge of: THPP-NMD regulations, AB 12, Title 22, trauma-informed care, and youth development best practices.
- Skills: Strong case management, crisis intervention, advocacy, and communication skills.
- Other: Must pass a background check, have a valid driver’s license, and reliable transportation.
Work Environment & Schedule:
- Full-time position with flexible hours, including occasional evenings or weekends as needed.
- Regular travel for home visits, meetings, and community-based services.
Pay: $25.43 - $35.00 per hour
