Job Description:
Blue Haven Memory Care is a small, intimate residential care provider with communities in Independence and Dallas, Oregon, dedicated to providing compassionate, person-centered care to residents living with Alzheimer's disease and other forms of dementia. Each community is home to just 16 residents, allowing for truly individualized attention and meaningful relationships. Our close-knit team environment allows nurses to know each resident personally while delivering high-quality care.
We are seeking a skilled and compassionate Registered Nurse to join our memory care team. This position will serve both our Independence and Dallas communities, with time split between the two locations. As an RN in our smaller facilities, you will know each resident by name and story, making a significant impact on their quality of life.
Responsibilities:
- Conduct comprehensive resident assessments to monitor health status, cognitive changes, and care needs
- Assist administrator with developing, implementing, and evaluating individualized care plans
- Ensure medication technicians are trained to administer medications and treatments safely and accurately
- Ensure medications are properly entered into the MAR and that care staff complete charting correctly
- Supervise care staff while maintaining accountability for resident outcomes (no delegation required)
- Train and educate staff to provide the highest quality care possible
- Assess and manage acute and chronic health conditions, including behavioral symptoms associated with dementia
- Respond to medical emergencies and coordinate hospital transfers when necessary
- Communicate effectively with administration and staff regarding care plans and health changes
- Maintain accurate documentation in accordance with state regulations
- Assist with admissions, discharges, and care transitions
- Provide occasional hands-on caregiving support as needed
- Travel between Independence and Dallas locations as scheduled
Qualifications:
- Current, unrestricted RN license in the state of Oregon
- 1-2 years of nursing experience preferred; memory care, residential care, or geriatric experience a plus
- Knowledge of Alzheimer's disease, dementia care, and person-centered care approaches
- Strong clinical assessment and critical thinking skills
- Ability to lead and mentor care staff
- Compassionate, patient demeanor when managing dementia-related behaviors
- Current CPR/BLS certification
- Experience implementing a new eMAR system preferred
- Reliable transportation to travel between locations
Schedule:
- Full-time
- 8-hour shifts
- Flexible scheduling to accommodate personal needs, with resident care needs taking priority
- Time split between Independence and Dallas locations, based on resident acuity at each community
