TriSource is seeking an experienced Manager of Trauma Case Management to join a nationally recognized healthcare organization in Houston. This is an exciting opportunity for a collaborative leader to oversee a dedicated case management team focused on delivering exceptional patient outcomes through care coordination, utilization management, and discharge planning.
In this role, you'll partner with physicians, nursing leadership, and interdisciplinary teams to improve quality outcomes, optimize resource utilization, and support patients throughout their hospital journey. Qualifications
- Current Texas RN license or Licensed Master Social Worker (LMSW); LCSW preferred
- Case Manager Certification (required)
- 5+ years of experience in case management, utilization management, discharge planning, or a related healthcare setting
- 3+ years of hospital-based nursing or social work experience
- 3+ years of leadership experience
- Bachelor's degree required; Master's degree preferred
