The Director of Medical Practice - NP/PA (DMP), along with providing primary care services to patients, works collaboratively with all members of the care team and leads practice site clinicians to enable best in class medical service to the patients of Martin’s Point. The practice site clinicians will report directly to the DMP. The DMP not only manages the care of a panel of patients by using best practices to diagnose and treat a variety of diseases and injuries, supporting call coverage for the practice, they also are responsible for managing all clinicians in their designated practice in conjunction with practice site operations leadership. The primary focus of the DMP is to provide clinical leadership to their designated site(s), to be the clinical dyad partner to the Director of Practice Operations (DPO), and to advise, assess and manage clinician performance with respect to quality of care, patient access and experience and value-based care performance. The DMP will be successful through the development of strong partnerships with clinicians, support staff, and other Martin’s Point Delivery System and Health Plan leaders. The DMP will model Martin’s Point values and create an engaging clinical culture that supports primary care, collaborative decision-making, outstanding customer service, continuous improvement, and the highest quality delivery of health care within a sound and sustainable business model.
PRIMARY DUTIES AND RESPONSIBILITIES

Ensures that all patients have outstanding access to high-quality, patient-centric care and education.

In partnership with the DPO, provides support and guidance to the clinicians in their site(s) with respect to clinical as well as business performance. This could include discussions and direction in terms of budget, scheduling and productivity expectations.

Provides direct oversight and guidance to the clinicians in their site(s) on ADDC, value-based care and quality performance.

Implements and supports clinical operations improvement opportunities in partnership with the Operations team.

Engages in activities at the health care center to assure compliance with appropriate standards required by all government agencies and other policies, regulations, and procedures required by MPHC.

Owns resolution of escalated clinical concerns.

Monitors patient complaints and collaborates with the Director of Practice Operations on clinical incidents, ensuring incident reports and follow up activities take place as indicated

Drives continuous improvement of patient care by ensuring ongoing assessment of ongoing projects

Actively engages in the recruitment process of new clinicians.

Owns the onboarding activities for new providers.

Responsible for clinician performance management

Responsible for clinician experience and retention

Conducts regular reviews of patient charts to ensure compliance with agreed-upon standards of care. This may be delegated with supervision to other clinicians seeking leadership and training opportunities.

Oversees chart and case review and helps to define scope and expectations of clinicians

Provides medical services as directed by MPHC.

The DMP will perform other duties as requested by MPHC consistent with their position as a leader at MPHC.

Maintains excellence of care delivery and uses MPHC’s leadership framework as their standard of effective leadership.

Leads Quality Improvement initiatives and development at their site and in collaboration with DPOs and other Delivery System leaders

Supports and drives Population Health initiatives and VBC contract goals

Leads clinician monthly meetings in conjunction with the DPO including agenda items related to quality, finances and other relevant topics.

Participates in daily huddles and regular team meetings to improve workflows and contribute to improving patient population outcomes

Responsible for clinician career and leadership development, coaching, mentoring, active listening and communication updates to team members. Provides educational oversight of clinician learners within the site

Reviews and discusses clinician and delivery system value-based care performance on a consistent basis

POSITION QUALIFICATIONS
Education:
-
Master's or doctoral degree from an accredited Nurse Practitioner program or Master's degree from an accredited Physician Assistant (Physician Associate) program.
Licensure/certification:
-
Board certification as NP or PA certification.

Active and unrestricted licenses in both ME and NH.
-
DEA license.

Active BLS or ACLS Certification
Experience:
-
3+ years of direct patient care experience required
-
Demonstrated experience working with Clinicians to deploy best clinical practices required

Prior management and/or leadership training is preferred
Pay Range: $198,658.60 - $245,401.80 The pay range above reflects the anticipated base pay range based on a full-time position. Actual compensation will be determined based on factors such as experience, skills, qualifications, and other job-related considerations. Employees may also be eligible for additional compensation, including incentive or commission-based programs, where applicable and subject to the terms of the relevant plan.
