Duties
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary
- Monitor and observe patients’ conditions by recording vital signs, noting changes in health status, and reporting findings accurately in electronic health record (EHR) or electronic medical record (EMR) system.
Qualifications
- Minimum 2 years experience as a CNA in Home Health Care ( not private duty)
- Minimum 1 year of home health care experience with Medicare Patients ( not private duty )
- Valid driver’s license and reliable insured transportation
- Knowledge of Medicare documentation requirements
- Proven experience working as a Home Health Care Aide or in assisted living, nursing homes, or long-term care settings
- Strong knowledge of medical terminology, medication administration protocols, catheter care, and vital signs monitoring
- Demonstrated ability to work effectively with individuals with disabilities or developmental challenges including dementia or geriatrics
- Familiarity with home health agency operations, home & community care practices, and relevant regulations such as HIPAA and home healthcare standards
- Certification in First Aid and Basic Life Support (BLS), including CPR training; additional certifications such as Hoyer lift operation or hospice care experience are a plus
- Excellent patient communication skills coupled with compassionate caregiving abilities to foster trust and rapport
- Physical ability to perform heavy lifting when necessary and assist with patient transfers using appropriate methods and equipment

Pay: $25.00 - $30.00 per hour
