Clinical Assessment Manager (UAS Experience Required)
Location: Bronx, NY / Westchester County, NY / Brooklyn, NY (Orientation in Bronx)
Pay Rate: $60–$65/hour
Job Type: Full-Time Contract
Schedule: Monday–Friday | 8:30 AM–5:30 PM
Hours: 40 Hours Weekly
Contract Type: Temp Assignment (Potential for Permanent Hire)
Job Overview
We are seeking experienced Clinical Assessment Managers with UAS experience for a full-time field-based contract assignment. This role focuses on conducting in-home clinical assessments, reassessments, and post-discharge evaluations across multiple New York service areas, including the Bronx, Westchester, and Brooklyn.
Key Responsibilities
- Conduct UAS assessments in accordance with state and organizational standards
- Complete clinical onboarding, reassessments, and comprehensive evaluations
- Perform medication reviews and post-hospital discharge assessments
- Provide accurate and timely clinical documentation for all participant encounters
- Ensure compliance with assessment protocols and care management guidelines
- Collaborate with interdisciplinary teams to support participant outcomes
- Conduct in-home visits across assigned boroughs and counties
- Identify changes in patient condition and escalate as clinically appropriate
Qualifications
Education
- Active Registered Nurse (RN) license in New York State required
- BS in Nursing or other health-related field preferred
Experience
- 3–5 years of experience in MLTC, LTHHCP, MAP, PACE, or other managed care programs required
- Minimum of 5+ years of experience in care management, case management, disease management, population health management, utilization review, quality assurance, or discharge planning (managed care, home care, or community health setting preferred)
- Minimum of 1 year experience working with frail or elderly populations (or completion of required PACE onboarding training if under 1 year experience but otherwise qualified)
- Proven ability to work independently with minimal supervision
Skills & Systems
- Proficiency in UAS and Lenavi required
- Experience with CRM platforms and clinical management systems
- Strong clinical assessment, documentation, and analytical skills
- Ability to manage field-based workflow across multiple locations
