Three Rivers Hospice is seeking a compassionate and experienced Bereavement Coordinator to join our St. Joseph team. This is an opportunity to make a lasting impact by supporting families through one of life's most difficult transitions.
Our Bereavement Coordinator provides grief counseling, emotional support, education, and follow-up services to families before and after the loss of a loved one. We are looking for someone who is empathetic, an excellent communicator, and passionate about helping others navigate the grieving process.
What You'll Do
- Provide individual grief counseling and emotional support to hospice families.
- Assess bereavement needs and develop individualized plans of care.
- Conduct bereavement follow-up visits, phone calls, and mailings in accordance with hospice regulations.
- Facilitate grief support groups and educational programs for families and the community.
- Provide grief education and resources to hospice staff, facilities, and community partners.
- Maintain accurate documentation and ensure compliance with hospice bereavement standards.
- Collaborate with the interdisciplinary hospice team to provide holistic support for patients and families.
Qualifications
- Experience in grief counseling, bereavement support, or counseling services is required.
- Bachelor of Social Work (BSW) or Chaplain experience preferred.
- Degree in Social Work, Counseling, Psychology, Ministry, Theology, or a related field is strongly preferred.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently while managing a flexible schedule.
- Valid driver's license, reliable transportation, and current auto insurance.
