*Job Summary*
We are seeking a dedicated Case Manager to join our team. The Case Manager will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary services and resources to improve their quality of life.
*Responsibilities*
- Conduct assessments to identify clients' needs and develop individualized care plans
- Coordinate and monitor services to ensure clients' needs are met effectively
- Provide crisis intervention and support during challenging situations
- Collaborate with other professionals and agencies to advocate for clients and access appropriate services
- Maintain accurate and up-to-date client records and documentation
- Utilize motivational interviewing techniques to empower clients towards positive change
- Provide mentoring and support to clients in areas such as support for aftercare services.
*Qualifications*
- Bachelor's degree in Social Work or related field (preferred not required)
- Proven experience in crisis management, addiction counseling, or related fields
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Certification in crisis intervention or behavioral health is a plus
- Demonstrated ability to work collaboratively with diverse populations
