As part of New York State’s 1115 Medicaid Waiver, Vita Valens helps to identify and address health-related social needs (HRSNs) by screening Medicaid members and directing them to community-based resources and care supports for housing, nutrition and transportation.
Job Overview
The Outreach Coordinator will serve as the face of Vita Valens in communities across Brooklyn, Queens, and Manhattan, building relationships and connecting Medicaid members to social care resources. This role is responsible for identifying, coordinating, and attending community events to engage Medicaid-eligible individuals in meaningful conversations and connect them with the 1115 Waiver’s social care resources.
Success in this role requires an outgoing, personable individual who enjoys meeting new people, networking with community organizations, and making individuals feel comfortable discussing their needs. In addition to community outreach, the Outreach Coordinator will conduct health-related social needs (HRSN) screenings, educate members about available benefits and services, and facilitate referrals to appropriate community resources. This position is primarily field-based, with travel throughout Brooklyn, Queens, and Manhattan and occasional office and remote work.
Responsibilities
- Identify, research, and coordinate participation in community events, health fairs, resource fairs, and other outreach opportunities that serve Medicaid populations.
- Adhere to all Vita Valens, SCN, Medicaid, HIPAA, and confidentiality policies and procedures.
- Build and maintain relationships with community organizations, faith-based organizations, healthcare providers, housing organizations, and other local partners to expand outreach opportunities.
- Represent Vita Valens at community events, engaging attendees in a warm, professional, and culturally sensitive manner.
- Initiate conversations with Medicaid members, educate them about available services, and collect contact information for follow-up by the Social Care team.
- Conduct Health-Related Social Needs (HRSN) screenings and assess eligibility for enhanced care services and community-based programs, and submit and manage documentation through the Social Care Network (SCN) platform, Unite Us, as needed.
- Provide education on benefits and resources available through the New York State Medicaid program and the Social Care Network.
- Maintain accurate, timely documentation of outreach activities, member interactions, screenings, referrals, and event outcomes.
- Meet outreach, engagement, and productivity goals while maintaining high-quality member interactions.
- Travel regularly throughout Brooklyn, Queens, and Manhattan to attend scheduled outreach events, meetings, and community networking opportunities, including occasional evenings or weekends as needed.
- Participate in team meetings, trainings, and ongoing professional development, including all required SCN certifications, HIPAA, and Mandated Reporter training.
Requirements
- Bachelor's degree completed.
- Passion for community engagement and improving health outcomes for underserved populations.
- Friendly, outgoing personality with exceptional interpersonal skills and the ability to quickly build rapport with diverse individuals.
- Strong networking and relationship-building abilities, with confidence initiating conversations and developing community partnerships.
- Excellent verbal communication, active listening, and customer service skills; bilingual abilities are highly preferred.
- Comfortable working independently in the field while effectively collaborating with an interdisciplinary team.
- Willingness and ability to travel frequently throughout Brooklyn, Queens, and Manhattan to attend community events, with some in-person office work
- Flexible schedule with availability to work occasional evenings and weekends based on outreach events.
- Strong organizational skills with attention to detail and the ability to accurately document member interactions and outreach activities.
- Technological proficiency, including experience with data entry and electronic documentation systems.
- Ability to maintain professionalism and confidentiality when handling sensitive member information.
Pay: $30.00 - $35.00 per hour
