Job Overview
We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team at The Rehabilitation Center at Jefferson Hills. In this vital role, you will assess, develop, and implement personalized treatment plans to improve patient mobility, reduce pain, and promote overall well-being. Your expertise will help patients regain independence across various settings, including outpatient clinics, hospitals, inpatient units, and home health environments. Duties
- Conduct comprehensive patient assessments to determine physical therapy needs, including evaluating physiology, anatomy, and medical history.
- Develop and implement individualized treatment plans utilizing evidence-based practices such as physiotherapy techniques, trigger point therapy, and orthopedics.
- Manage discharge planning processes to ensure seamless transitions from hospital or inpatient care to outpatient or community settings.
- Document all patient interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards.
- Collaborate closely with healthcare professionals—including physicians, nurses, and occupational therapists—to optimize patient outcomes through coordinated care plans.
- Provide patient rehabilitation services tailored for diverse populations such as geriatrics, pediatrics, neurology patients, and those requiring home & community care.
- Educate patients and their families on treatment procedures, post-discharge care plans, and injury prevention strategies to foster active participation in recovery.
Experience
- Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, inpatient units, or home health environments.
- Demonstrated expertise in acute care settings with knowledge of discharge planning and inpatient rehabilitation processes.
- Familiarity with outpatient rehabilitation center operations and outpatient clinic experience is highly desirable.
- Strong understanding of medical terminology, physiology, anatomy, and treatment modalities specific to orthopedics and neurology.
- Experience working with geriatric patients and pediatric populations to address unique therapeutic needs effectively.
- Proficiency in managing electronic health records (EHR) or electronic medical records (EMR) systems while maintaining compliance with clinical documentation standards.
- Ability to collaborate effectively with healthcare teams to develop comprehensive treatment plans that meet individual patient goals.

Pay: $43.00 - $48.00 per hour
