Duties
- Assess patients’ physical conditions and develop individualized treatment plans aligned with clinical best practices and patient goals.
- Deliver hands-on physical therapy interventions, including therapeutic exercises, manual therapy, trigger point therapy, and mobility training.
- Coordinate discharge planning and collaborate with healthcare professionals such as physicians, nurses, and occupational therapists to ensure seamless patient care transitions.
- Maintain accurate clinical documentation standards within electronic health records (EHR) systems, ensuring compliance with regulatory requirements.
- Conduct ongoing evaluations of patient progress and adjust treatment plans accordingly to optimize recovery outcomes.
- Educate patients and their families on home exercise programs, safety measures, and community resources to support ongoing rehabilitation.
- Manage home & community care logistics efficiently while adhering to home health protocols and safety guidelines.
Skills
- Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health settings.
- Strong knowledge of physiology, anatomy, orthopedics, neurology, geriatrics, pediatrics, and related medical terminology.
- Expertise in treatment plan development for diverse populations including geriatric patients and those recovering from orthopedic or neurological conditions.
- Familiarity with outpatient rehabilitation center operations and outpatient clinic experience.
- Skilled in discharge planning processes and working collaboratively with multidisciplinary healthcare teams.
- Proficiency managing electronic health records (EHR) or electronic medical records (EMR) systems for accurate documentation of patient care activities.
- Ability to communicate effectively with clients of varying ages and backgrounds while demonstrating empathy and professionalism.
- Experience with inpatient hospital settings or inpatient rehabilitation units is highly desirable.
- Knowledge of physiotherapy techniques such as trigger point therapy and patient rehabilitation strategies for complex cases.
- Working understanding of home & community care environments to facilitate safe and effective treatment delivery.

Pay: $60.00 - $70.00 per hour
