Home Health CM needed for Palm Harbor Florida
Must have Case Manager Home Health experience 2 years min
Required Skills & Experience: -Minimum of one (1) year of related experience and/or training. OR -Equivalent combination of education and experience. Preferred Skills & Experience: -Prefer one (1) year experience in an acute or post-acute care setting. -Preferred experience caring for terminal patients and others with life threatening illnesses. -Ability to type 25 wpm is highly preferred.
