The LPN plays a key role in ensuring seamless patient care throughout neurosurgical care plan. This position is responsible for assisting with FMLA/disability paperwork and supporting providers with clinical care management activities. The ideal candidate is an excellent communicator who can effectively prioritize patient needs, collaborate with providers, and maintain high standards of patient service and documentation.
Partners with other care team members to:
- Complete and process FMLA and disability paperwork.
- Perform telephone triage for assigned patients, assess concerns, and escalate clinical issues appropriately.
- Conduct post-procedure discharge and follow-up calls to assess patient status and address concerns.
- Support pre- and post-injection patient management when scheduled in the injection suite.
- Collaborate with providers and clinical staff to optimize patient outcomes.
- Maintain compliance with HIPAA and organizational policies.
- Support quality initiatives and patient satisfaction efforts.
