Job Summary
We are seeking dedicated Lead Care Managers to support our Enhanced Care Management (ECM) initiative. As a Lead Care Manager in Enhanced Care Management, you will serve as a central resource for clients navigating complex health and social services. The role focuses on coordinating and overseeing care plans for individuals with high needs, including those experiencing homelessness or chronic health issues. Lead Care Managers work closely with clients, families, and healthcare providers to ensure cohesive, individualized support.
Responsibilities
- Care Coordination and Planning
- Develop, implement, and manage individualized care plans that incorporate the physical, behavioral, and social health needs of clients.
- Conduct thorough assessments to identify each client's specific needs, resources, and challenges.
- Collaborate with clients, families, and interdisciplinary teams to set measurable goals for health improvement.
- Client Engagement and Advocacy
- Engage clients regularly to encourage self-management and accountability in meeting health and life goals.
- Act as a primary advocate for clients, ensuring their voices are heard and respected in all care decisions.
- Educate clients on community resources, benefits, and services they can access to improve their quality of life.
- Documentation and Reporting
- Maintain accurate, detailed records of client interactions, progress, and changes in status.
- Use case management software to document all care activities and ensure data compliance with program standards.
- Prepare regular progress reports for supervisors and contribute to quality improvement efforts.
- Team Collaboration
- Participate in interdisciplinary team meetings, contributing valuable insights on client needs and case progress.
- Work closely with healthcare providers and social service agencies to streamline care coordination.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Nursing, or a related field (Master’s preferred).
- Minimum of 2 years of experience in case management, social services, or a related field.
- In-depth knowledge of local health and social service resources in Contra Costa County.
- Strong proficiency in care coordination principles, electronic health records, and case management software.
- Excellent communication, interpersonal, and organizational skills; bilingual abilities are a plus.
- Valid driver’s license and reliable transportation.
