Duties and Responsibilities:
- Providing interventions to include self help skill development, system advocacy, individual advocacy, pre-crisis and post crisis support, housing, education/employment, and meals and social activities
- Provide input into the Person-Centered Plan
- Assist with enhancement of development of natural supports, as well as coping and self management skills of the individuals served
- Provide supportive services to assist an individual in community reentry following hospitalization.
- Assist individuals with the acquisition, development and expansion of rehabilitative skills needed to move forward in recovery
- Assisting individuals with development of system advocacy, individual advocacy, pre-crisis and post-crisis support skills
- Provide individual with various skill building activities
- Assisting with development of daily and community living skills
- Assisting individual with development of socialization skills
- Assisting individual with development of leisure time, interests/activities
- Monitoring for progress/lack of progress through observations, interview, and documentation review.
- Providing education on mental health/substance abuse related issues
- Assisting with behavior and feelings management skills
