The Adams County Health Department is seeking a compassionate, organized, and motivated Maternal/Child Health Family Navigatorto join our Better Birth Outcomes – Comprehensive Program. This position works one-on-one with pregnant individuals and families with young children to help them identify goals, overcome barriers, and connect with community resources that improve health and well-being. The ideal candidate enjoys building relationships, empowering families, coordinating services, and helping others navigate complex systems such as healthcare, Medicaid Managed Care Organizations (MCOs), and community support programs.
Description of Position
The Maternal/Child Health Family Navigator serves as a trusted partner for families enrolled in the Better Birth Outcomes – Comprehensive Program. Using a strengths-based and motivational interviewing approach, the Navigator assists families in identifying their goals, addressing barriers, and connecting to healthcare and community resources that support healthy pregnancies, healthy babies, and thriving families.The Navigator works closely with participants to improve access to medical care, Managed Care Organization (MCO) benefits, community programs, transportation, housing resources, nutrition services, childcare resources, and other supports based on each family's individual needs.
General Duties
- Build supportive relationships with pregnant individuals and families through home visits, office visits, telephone calls, and community outreach.
- Complete assessments to identify health-related social needs and barriers to care.
- Assist families in establishing personal health and wellness goals and monitor progress toward those goals.
- Provide education regarding available community resources and connect families with appropriate services.
- Assist participants in understanding and accessing Medicaid Managed Care Organization (MCO) benefits and value-added services.
- Encourage and support establishment and maintenance of a medical home.
- Coordinate referrals to community agencies and healthcare providers.
- Collaborate with community partners to ensure families receive comprehensive support.
- Utilize motivational interviewing and shared decision-making techniques to empower participants.
- Maintain accurate electronic and written documentation in accordance with program requirements.
- Participate in required trainings, meetings, and quality improvement activities.
- Maintain confidentiality and comply with all applicable program policies and regulations.
- Perform other duties as assigned.
Qualifications
- Applicants must possess one of the following: associate degree in behavioral science, social science, public health, human services, health education, psychology, sociology, social work, or another health-related field.
Pay: From $41,600.00 per year
