What You'll Be DoingDeliver Community-Based Primary Care
- Conduct provider visits throughout the Willmar region
- Perform comprehensive patient assessments and ongoing clinical evaluations
- Diagnose and manage acute and chronic medical conditions
- Develop individualized treatment plans
- Identify changes in condition early and intervene proactively
Collaborate With an Outstanding Clinical Team
- Work alongside physicians, nurse practitioners, nurses, therapists, caregivers, and facility leadership
- Support chronic disease management and preventative care initiatives
- Educate patients and families regarding treatment plans and disease management
- Participate in specialty programs and quality improvement initiatives
Improve Patient Outcomes
- Reduce unnecessary emergency department utilization and hospital readmissions
- Strengthen continuity of care across healthcare settings
- Improve patient outcomes through proactive interventions
- Build long-term relationships with residents and interdisciplinary care teams

- Graduate of an accredited Physician Assistant program
- Active Minnesota PA-C license in good standing
- Strong clinical assessment and medical decision-making skills
- Ability to practice independently while collaborating with physicians and interdisciplinary teams

- Reliable transportation and willingness to travel throughout Western Minnesota
