Restorative Nurse

Responsibilities:
- Provide restorative care to patients, including rehabilitating and maintaining their physical, emotional, and social well-being
- Develop and implement individualized care plans, in collaboration with the interdisciplinary team
- Conduct comprehensive assessments, documenting patient progress and identifying areas for improvement
- Collaborate with peers and other healthcare professionals to deliver evidence-based care
- Promote patient education and empowerment, ensuring they are informed and involved in their care
- Maintain accurate and timely records, adhering to regulatory and organizational standards
Requirements:
- 2 years of experience as a Restorative Nurse (or equivalent experience in a related field)
- Current licensure as a Registered Nurse or LPN in the state of Illinois
- Possess strong communication, interpersonal, and organizational skills
- Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and responsibilities
