What You'll Be Doing
As a Community Liaison, you'll serve as the face of Legacy Home Care throughout the Twin Cities. You'll develop referral relationships, educate professionals and families, and create opportunities that drive census growth and strengthen the Legacy Home Care brand.
Responsibilities include:
- Prospecting and developing new referral relationships throughout the community.
- Calling on hospitals, clinics, senior living communities, social workers, discharge planners, and healthcare providers.
- Conducting drop-ins, presentations, and networking activities.
- Building trust with families and serving as a resource during the decision-making process.
- Maintaining an active pipeline and consistently following up on opportunities.
- Representing Legacy Home Care at professional events and within the community.
- Collaborating closely with operations and clinical teams to ensure exceptional customer experiences.
