Fairfield Memorial Hospital is seeking a compassionate, skilled Senior Life Solutions Primary Therapist to support the behavioral health needs of program participants. This role plays a vital part in delivering high‑quality, patient‑centered care by conducting psychosocial assessments, developing individualized treatment plans, and providing therapeutic interventions that promote emotional well‑being, safety, and dignity. The therapist works directly with patients across varying levels of need and ensures that all services reflect FMH’s commitment to excellence and compassionate care. In addition to providing individual, group, and family therapy, the Primary Therapist collaborates closely with interdisciplinary team members to coordinate care, support discharge planning, and maintain compliance with hospital, state, and federal standards. This position also contributes to quality improvement initiatives, supervises social work interns, and represents the program in community or educational settings. The therapist plays a key role in ensuring continuity of care and supporting the overall success of the Senior Life Solutions program.
Conduct psychosocial assessments and develop the therapy components of individualized treatment plans
Provide individual, group, and family therapy in accordance with patient needs and provider orders
Maintain accurate, timely clinical documentation and treatment updates
Coordinate discharge planning and ensure completion of all required documentation
Collaborate with physicians, nurses, and interdisciplinary team members to support patient goals
Uphold patient rights, confidentiality, dignity, and safety
Represent the program at educational or community events as requested
Assist with data collection, reporting, and quality improvement activities under the QAPI Plan
Supervise and mentor social work interns
