Overview
Trays Home Health Care LLC is seeking compassionate and dedicated Direct Support Professional(s) to provide high-quality assistance and care to individuals with diverse needs.

This paid position offers an opportunity to make a meaningful difference in the lives of those served by delivering personalized care in various settings such as group homes, assisted living facilities, persons served residence, and home health environments.
Responsibilities
- Assist individuals with Activities of Daily Living (ADLs)
- Observe for any changes in health or behavior, documenting findings accurately in compliance with HIPAA regulations. .
- Prepare nutritious meals tailored to individual dietary needs while maintaining cleanliness and safety standards in the kitchen areas.
- Support residents with memory care needs by implementing specialized strategies and ensuring a safe and comforting environment.
- Engage clients in social activities and community outings to promote socialization, independence, and overall well-being.
- Maintain a clean and safe environment by performing routine cleaning tasks and adhering to infection control procedures.
- Collaborate with healthcare teams, social workers, and family members to develop and update personalized care plans that reflect each individual's goals and needs.
Qualifications
- Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly desirable.
- Knowledge of patient observation techniques, and HIPAA compliance.
- Strong interpersonal skills with a compassionate approach toward working with individuals with disabilities or chronic health conditions.
- Experience working with individuals with developmental disabilities or autism is advantageous.
- Basic cooking skills for meal preparation that meets nutritional guidelines for various health conditions.
- A valid driver's license and insured vehicle are required for community outings or transportation assistance.
- Must be able to work Monday-Friday between the hours of 9am-5pm. No overnights. No weekends.
