Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
- Administer medications and injections accurately while adhering to safety protocols and documentation standards.
- Assist with complex procedures such as IV infusions, catheterizations, wound suturing, and sterile technique practices.
- Manage electronic health records (EHR/EHR systems) efficiently to ensure precise medical documentation and compliance with HIPAA regulations.
- Provide specialized care for patients with Alzheimer's disease, dementia, or memory impairments in memory or hospice care settings.
- Collaborate with interdisciplinary teams to develop and implement personalized care plans for patients with acute or chronic conditions.
- Educate patients and their families on health management strategies, medication use, and post-discharge instructions to promote recovery and independence.
Qualifications
- Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or region.
- Proven experience working in diverse healthcare environments such as long-term care facilities, hospitals (including ICU or trauma centers), assisted living communities, or home care settings.
- Strong knowledge of medical terminology, anatomy, physiology, and clinical procedures including IV insertion and catheter care.
- Familiarity with EMR/EHR systems like Cerner or Athenahealth for accurate medical documentation.
- Experience working with geriatric populations or individuals with disabilities; knowledge of dementia and Alzheimer's care is highly desirable.
- Ability to perform physical examinations, vital sign assessments, specimen collection/processing, and basic life support (BLS).
- Excellent communication skills for caregiver interaction, patient education, and team collaboration; sensitivity to patient confidentiality policies such as HIPAA is essential.
