Stephen Center has served homeless and low-income individuals in Omaha since 1984 with the mission to partner with the community, families and individuals to overcome homelessness, addiction and poverty. The Pettigrew Emergency Shelter and Permanent Supportive Housing offerings provide safe and secure housing while individuals and families complete an assessment of permanent housing options. All Stephen Center residents have access to dual diagnosis support services via the HERO addiction/recovery program. 100% of Stephen Center shelter supportive housing participants are living in poverty. Housing transition support identifies poverty barriers that may impact one’s ability to achieve or maintain permanent housing through case management, life skills, and behavioral health services. Stephen Center serves approximately 250 people daily in all programs.
Purpose:
The HERO Case Manager plays a critical role in supporting a coordinated, client-centered recovery environment by integrating clinical, operational, and community-based services. Working collaboratively across interdisciplinary teams, this position ensures safety, accountability, and continuity of care for individuals navigating co-occurring substance use and mental health recovery. The role focuses on connecting clients to appropriate referrals and resources that support both immediate needs and long-term recovery, while partnering with clinical staff, leadership, and administrative teams to maintain a safe and structured program environment.
Through ongoing assessment, the Case Manager identifies client needs related to physical and mental health, housing, employment, family support, and other essential services, and works closely with the HERO care team and community partners to implement individualized solutions. This position supports clients in preparing for independent living while ensuring all services, client engagement, and outcomes are accurately and timely documented in alignment with program standards, compliance requirements, and overall quality of care.
