Overview
Join our dynamic healthcare team as an RN Case Manager, where you will coordinate and oversee patient care across the continuum of care. In this essential role, you will collaborate with physicians, nurses, therapists, patients, families, and community resources to develop individualized care plans, facilitate safe transitions, and promote positive health outcomes. Your clinical expertise and care coordination skills will help ensure patients receive high-quality, cost-effective, and patient-centered care while improving their overall quality of life.
Duties
- Conduct comprehensive patient assessments to evaluate medical, psychosocial, functional, and discharge planning needs.
- Develop, implement, and monitor individualized care plans to support optimal patient outcomes.
- Coordinate care with physicians, specialists, therapists, home health agencies, skilled nursing facilities, and community resources.
- Facilitate transitions of care, including hospital admissions, discharges, referrals, and follow-up services.
- Review treatment plans and monitor patient progress, making recommendations to address barriers to recovery and improve care coordination.
- Educate patients and families regarding diagnoses, medications, treatment plans, disease management, and available healthcare resources.
- Collaborate with interdisciplinary teams to ensure timely, appropriate, and cost-effective care delivery.
- Advocate for patients by identifying and resolving barriers to care, including insurance, transportation, and access to community services.
- Maintain accurate, timely documentation in EMR/EHR systems such as Epic or Cerner while ensuring compliance with HIPAA, CMS, and organizational policies.
- Participate in utilization review, discharge planning, quality improvement initiatives, and regulatory compliance activities as needed.
Requirements
- Active, unrestricted Registered Nurse (RN) license in the state of practice.
- Current Basic Life Support (BLS) certification.
- Minimum of 2 years of clinical nursing experience; prior case management, care coordination, utilization management, discharge planning, home health, hospice, or acute care experience preferred.
- Strong knowledge of Medicare, Medicaid, commercial insurance, and care coordination principles.
- Excellent assessment, critical thinking, communication, organizational, and problem-solving skills.
- Experience using EMR/EHR systems, including Epic or Cerner, and proficiency with Microsoft Office applications.
- Ability to manage multiple priorities, work independently, and collaborate effectively within interdisciplinary healthcare teams.
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification preferred or willingness to obtain within the required timeframe.
Pay: $2,500.00 - $3,000.00 per week
