Thrive Healthcare is a premier home health agency dedicated to clinical excellence and a seamless experience for our patients. We are looking for a meticulous and driven Intake & Care Coordinator to join our team.
The Team Structure
You will be a key member of a high-functioning Care Coordination Team of three. While you will be responsible for managing your local office, you will work in close, daily collaboration with two other Coordinators based in our regional locations. We operate as a tight-knit, agile team where everyone works together collaboratively—ensuring that even across different offices, we function as a single unit to support our patients and clinicians.
Key Responsibilities
Intake & Admissions
- Referral Management: Process incoming referrals from hospitals, clinics, and physicians with urgency and accuracy.
- Insurance Verification: Verify Medicare, Medi-Cal, and private insurance benefits; obtain initial authorizations.
- Compliance: Ensure all data entry meets California Title 22 and HIPAA standards.
Care Coordination & Recruitment
- Scheduling: Coordinate Start of Care (SOC) visits between patients and the clinical team (RNs, PTs, OTs).
- Caregiver Hiring: Manage the full-cycle recruitment of Home Care Aides (HCAs). This includes:
- Posting ads and screening candidates for clinical judgment and empathy.
- Verifying PER IDs on the California Home Care Aide Registry.
- Managing the Live Scan and Guardian portal background check process.
- Staff Compliance: Track the "compliance calendar" to ensure all field staff have updated Annual TB Screenings and Health Acknowledgments on file.
Qualifications
- Experience: Minimum 2 years in staffing or in healthcare
- Collaboration: Proven ability to work effectively within a distributed or multi-office team.
- Regulatory Knowledge: Familiarity with California Title 22 and the Home Care Services Consumer Protection Act.
- Tech Savvy: Proficiency in using software and attention to detail
