Responsibilities
- Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts when necessary
- Monitor and observe patients’ health status by recording vital signs and reporting any changes promptly
- Administer medications accurately following medical instructions and document all medical records in Electronic Health Record (EHR) systems while maintaining HIPAA compliance
- Support patients with specialized needs including dementia, Alzheimer’s, memory care, or developmental disabilities through tailored activities and attentive care
- Provide assistance with medical procedures such as catheter care, wound care, and hospice support under supervision of healthcare professionals
- Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and safety protocols
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall well-being
- Document patient interactions diligently using medical documentation standards and ensure confidentiality of all medical records
Experience
- Prior experience working in assisted living facilities, nursing homes, or home health environments is highly desirable
- Demonstrated ability to work with individuals with disabilities or cognitive impairments such as dementia or Alzheimer’s disease
- Familiarity with EMR (Electronic Medical Record) or EHR systems for accurate documentation of patient care activities
- Knowledge of medical terminology relevant to home health caregiving roles
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are a plus
- Experience using assistive devices like Hoyer lifts or working with patients requiring long-term or hospice care is advantageous
- Strong communication skills to collaborate effectively with healthcare teams and family members
