Job Title: Diabetes Care Registered Nurse (RN)
Job Summary
We are seeking an experienced Registered Nurse (RN) with a strong background in diabetes management and patient education to join an outpatient diabetes care team. The RN will play a key role in providing comprehensive diabetes education, medication management, device training, and care coordination while collaborating with endocrinologists, nurse practitioners, dietitians, health coaches, and other interdisciplinary team members. The ideal candidate has experience serving as a lead RN in an outpatient setting and is confident managing patients with complex diabetes care needs.
Key Responsibilities
- Provide comprehensive education on diabetes disease management, including lifestyle modifications and self-management strategies.
- Educate patients on diabetes medications, including administration techniques, expected outcomes, and potential side effects.
- Assist with prior authorizations for diabetes medications and coordinate appeals for insurance denials.
- Collaborate with pharmacies to resolve medication coverage issues and identify appropriate therapeutic alternatives.
- Perform glucose tolerance testing while ensuring patient safety and accurate test results.
- Teach insulin administration, management of hyperglycemia and hypoglycemia, sick-day guidelines, and glucagon pen usage.
- Train patients on the use, troubleshooting, and maintenance of continuous glucose monitors (CGMs) and insulin pumps.
- Upload and review data from home glucose monitoring devices to support both in-person and virtual patient visits.
- Coordinate insulin pump authorizations with vendors and provide ongoing technical and emotional support.
- Ensure patients maintain adequate diabetes medication and supply inventories to prevent interruptions in treatment.
- Provide culturally sensitive nutrition education, including dietary approaches such as the Mediterranean diet.
- Develop individualized exercise recommendations based on patient needs and health status.
- Respond to patient inquiries regarding diabetes management and treatment plans.
- Identify patients experiencing diabetes-related distress and coordinate referrals with appropriate care team members.
- Collaborate closely with endocrinologists, nurse practitioners, registered dietitians, health coaches, and behavioral health professionals to provide comprehensive patient care.
- Maintain accurate documentation within the electronic medical record (EMR).
Required Qualifications
- Bachelor of Science in Nursing (BSN).
- Active and unrestricted New York State Registered Nurse (RN) License.
- Certification in Diabetes Education.
- Strong RN experience with the ability to function as a lead nurse.
- Previous outpatient nursing experience.
- Demonstrated experience providing diabetes education.
- Hands-on experience with insulin pumps and continuous glucose monitoring (CGM) systems.
- Proficiency with electronic health records (EHR/EMR) and medical software.
- Excellent patient education, communication, and interpersonal skills.
Preferred Qualifications
- Master's degree in Nursing or a related field.
- Experience working in endocrinology or diabetes specialty clinics.
Schedule
- Monday, Wednesday, Friday: 8:30 AM – 5:00 PM
- Tuesday & Thursday: 8:30 AM – 7:00 PM
- One to two Saturdays per month: 8:00 AM – 4:30 PM
- 40 hours per week.

Pay: $65.00 - $70.00 per hour
