Position: (Community Support Team) Certified Peer Support Specialist
Summary: Community Support Team services provides supports to adults with a Diagnostic and Statistical Manual of Mentel Disorders Fifth Edition (DSM-5) diagnosis of mental illness, substance use, or co-morbid disorder and who have complex and extensive treatment needs. This service consists of community-based mental health and substance use services, and structured rehabilitative interventions intended to increase and restore a beneficiary’s ability to live successfully in the community. The team approach involves structured, face-to-face therapeutic interventions that assist in reestablishing the beneficiary’s community roles related to the following life domains: emotional, behavioral, social, safety, housing, medical and health, educational, vocational, and legal. This team service includes a variety of interventions that are available 24-hours-a-day, 7-days-a-week, 365-days-a-year. The service is designed to provide: (1) symptom stability by reducing presenting psychiatric or substance use disorder symptoms; (2) restoration interventions for development of interpersonal, community, coping and independent living skills; (3) psychoeducation; (4) first responder intervention to deescalate a crisis; and (5) service coordination and ensure linkage to community services and resources.
Job Roles, Duties and Responsibilities
· Serves as an active member of the Community Support Team
· Provides interventions according to the individualized Person-Centered Plan
· Approaches position as a role model, versus an expert authority
· Practices under supervision according to the service definition
· Documents services rendered in a progress note in EHR
· Participates in team meetings at least weekly
· Provides input into the person-centered planning process
· Guides and encourages recipients to take responsibility for and actively participate in their own recovery
· Models recovery values, attitudes, beliefs, and personal action to encourage wellness and resilience
· Teaches and promotes self-advocacy to the recipient
· Supports and empowers the recipient to exercise their legal rights within the community
· Participates in monthly supervision
· Attends staff meetings
· Participates in required trainings
· Follows NC Peer Support Certification Guidelines under the Behavioral Health Resource Plan, School of Social Work, UNC Chapel Hill
· Performs other reasonable tasks and assignment relevant to assist recipient in accomplishing goals or crisis prevention as delegated by supervisor
Minimum qualifications:
- Must have a high school diploma or GED equivalency.
- Must be registered as a North Carolina Certified Peer Support Specialist.
- 1 year of experience working with adults in with substance use and/or mental health disorders.
