Penn Medical Home Health Care is seeking a compassionate, detail-oriented PRN Registered Nurse (RN) to perform Start of Care (SOC) and Recertification visits for pediatric home health patients. This flexible, field-based position is ideal for RN who values autonomy, meaningful patient relationships, and work-life balance.
What You'll Do
- Conduct comprehensive Start of Care (SOC) and Recertification nursing assessments in patients' homes.
- Complete accurate clinical evaluations and develop individualized documentation based on patient needs.
- Prepare and submit the Plan of Care (POC/485) for physician review and signature.
- Ensure all clinical documentation is completed accurately, timely, and in compliance with Medicare, Medicaid, ACHC, and agency standards.
- Collaborate with physicians, clinical staff, and the interdisciplinary care team to ensure continuity of care.
- Identify changes in patient condition and communicate clinical findings promptly.
- Maintain organized, audit-ready patient records while protecting patient confidentiality.
- Deliver compassionate, patient-centered care while following all agency policies and regulatory requirements.
Qualifications
- Active Pennsylvania Registered Nurse (RN) license in good standing.
- Excellent assessment, critical thinking, and clinical documentation skills.
- Strong organizational and time management abilities.
- Reliable transportation and willingness to travel within assigned service areas.
- Current CPR certification.
- Professional, compassionate, and effective communication skills.
