We are seeking a compassionate and dedicated Medical Social Worker to join our family-owned hospice agency. The ideal candidate will be responsible for providing support to patients and families facing end-of-life care. This role involves working closely with clients to assess their needs and facilitate access to resources and services.
Overview
IMMEDIATE HIRE Position
Hourly: $38-$45 per hour + paid mileage
Standard shift: Day shift
Weekly schedule: Monday to Friday
Ability to commute:
- Orange County, CA: Reliably commute or planning to relocate before starting work (Required)
- Los Angeles: Reliably commute or planning to relocate before starting work (Required)
License/Certification:
- Driver's License (Required)
- Masters in Medical Social Work (Required)
Willingness to travel:
- Visiting patients in private homes, board and cares, senior centers
Work Location: Los Angeles/ San Bernardino
Responsibilities
- Conduct comprehensive patient assessments to identify needs.
- Provide psychotherapy and counseling services to individuals focusing on mental health and emotional support.
- Collaborate with interdisciplinary team members to provide comprehensive hospice care.
- Advocate for clients by connecting them with community resources, including home care services and support groups.
- Facilitate individual and group counseling sessions tailored to the unique needs of clients.
- Maintain accurate records of client interactions, progress notes, and treatment plans in compliance with legal and ethical standards.
