Purpose: (General description and summary
The ICM Float Specialist provides community-based interventions designed to stabilize and support high-need individuals enrolled in Intensive Case Management (ICM) services in Multnomah County through an assertive engagement approach. Unlike traditional ICM case managers, the Float Specialist does not carry a designated caseload and instead serves as a flexible team resource to increase participant engagement, continuity of care, and service capacity across the program.
The Float Specialist works collaboratively with ICM case managers, mental health clinicians, and other team members to provide additional participant contacts, outreach, skill building, resource connection, transportation support, crisis response, and engagement services based on current program needs. The position is intended to increase program capacity and responsiveness by providing enhanced participant support, assisting with engagement of individuals, and offering additional service capacity during periods of increased participant need.
The Float Specialist also provides coverage for participants when primary staff are unavailable due to leave, illness, vacancies, training, or other operational needs. By maintaining participant engagement and supporting continuity of care during staffing disruptions, the position helps reduce service interruptions, mitigate risk, and maintain fidelity to the ICM model.
Services are delivered from a trauma-informed, person-centered, and equity-focused lens, emphasizing strong advocacy, community integration, and connection to resources and peer supports. The Float Specialist follows procedures, outcome goals, and reporting requirements as outlined by New Narrative's contract with Multnomah County and CareOregon. Services are primarily community-based and may include outreach at participant residences, hospitals, shelters, community locations, and appointments, with at least two-thirds of work time spent providing services in the field.
This is primarily community based outreach position where 60-75% of your time will be in the community in a mixture of direct care, coordination, skills training, transportation, or travel. You will be provided technology to support community based work. Due to the nature of the work, you will be exposed to various weather conditions and expected to maintain outreaching unless directed to otherwise by your supervisor. There is an office space at a clinic for administrative tasks and team meetings. This is direct care role that requires in person contact with participants.
Accountabilities: (Responsibilities of the job)
ICM Specialist will work collaboratively with Mental Health Clinicians, Clinical Case managers, and participant to create and implement the participant’s individual service plan. The ICM team provides comprehensive wrap around services through inter-disciplinary care to assist individuals to meet their basic needs, promote safety, accomplish goals, as outlined in the collaborative treatment plan, and attain the highest level of independence possible. These services included coordination of care, skills training, case management and connection to local benefits, coordination with medical providers (and other providers as needed) and assisting participants in meeting basic IADL needs. Number of contacts are weekly and community based in combination with coordination with providers and integrated team.
Assist participant in connecting to resources and maintaining resources needed to achieve goals set in the individual service and support plan, including: SSI/SSDI, SNAP, vital documents, ID, etc.
Demeanor: Remaining calm in crisis situation, empathetic, unbiased, non-judgmental and excellent peer model skills.
Experience/Qualifications:
A minimum of two years of experience in social work or related field preferred. Prior experience with SPMI, houseless population, and community based services preferred. Spanish/English desirable. Applicant must possess the skills necessary to work independently. Understanding of psychiatric medications and their use, Administrative rules and other applicable laws and regulations, and thorough understanding of acceptable clinical practices and guidelines.
Extensive mental activities required. Ex: Use of educated and clinical judgment, communicate thought clearly, maintain calm demeanor in crisis situations, be an empathetic and effective listener, flexible analytical and interpreting skills, etc. Ability to motivate others and remain consistently enthusiastic. Ability to self-regulate and independently manage emotions, stress, and burnout.
