Forest City Nursing and Rehab is seeking a dedicated PRN Speech-Language Pathologist (SLP) to provide high-quality therapy services in our skilled nursing environment. This is a meaningful opportunity to help residents regain communication, cognition, and swallowing function while working alongside a compassionate interdisciplinary care team.
Deliver speech-language pathology services in accordance with clinical standards and facility care protocols
Conduct comprehensive evaluations and collect data to support individualized treatment planning
Develop and implement treatment plans with measurable short- and long-term goals
Educate residents, families, and staff on therapy techniques and strategies to support carryover and daily function
Maintain accurate and timely documentation
Collaborate with physicians, nursing staff, and rehabilitation team members to ensure quality resident outcomes
Active Speech-Language Pathologist (SLP) license in the state of Pennsylvania
