Windsor Healthcare Residence is seeking a compassionate and dedicated Part-Time Licensed Social Worker to join our team!
Qualifications:
- Licensed Social Worker in good standing, as required by the State.
- Long-term care experience preferred.
- Strong interpersonal, organizational, and communication skills.
- Compassionate, resident-centered approach to care.
- Ability to work collaboratively with residents, families, and the interdisciplinary care team.
Responsibilities:
- Conduct resident social assessments and develop individualized care plans.
- Provide emotional support and counseling to residents and their families.
- Participate in interdisciplinary care plan meetings.
- Coordinate discharge planning and community resources.
- Assist residents and families with adjustment to the facility and available services.
- Maintain accurate documentation and ensure compliance with applicable regulations.
