Position Overview
As a Home Health Physical Therapist, you will:
Deliver individualized physical therapy services in patients’ homes or skilled nursing facilities

Evaluate patients and develop customized treatment plans

Monitor progress and adjust care plans as needed

Collaborate with physicians, caregivers, and interdisciplinary care teams

Help patients improve mobility, independence, and quality of life

Qualifications
Doctorate or Degree in Physical Therapy (DPT or PT)

- Minimum 2 years of clinical experience
Home Health or Skilled Nursing Facility experience preferred

Active California Physical Therapist License
-
Valid California Driver’s License
-
Current CPR certification
-
Current TB test and Professional Liability Insurance

Strong communication and organizational skills

Ability to work independently and manage a flexible caseload
