Position Type: Contract (6 Months to 1 Year)
Schedule: Full-Time | 40 Hours Guaranteed Weekly
Pay Rate: $65–$68 per hour
Join a dedicated rehabilitation team in a well-established Skilled Nursing Facility in Yountville, California. We are seeking a compassionate and motivated Physical Therapist who is passionate about helping patients regain mobility, improve functional independence, and enhance their quality of life.
Responsibilities:
- Evaluate and assess patients' physical therapy needs.
- Develop individualized treatment plans based on patient goals and clinical findings.
- Provide skilled therapeutic interventions to improve strength, balance, mobility, and function.
- Monitor patient progress and modify treatment plans as needed.
- Collaborate with physicians, nursing staff, and interdisciplinary team members.
- Maintain accurate and timely documentation in accordance with facility and regulatory requirements.
- Educate patients and caregivers on treatment plans and home exercise programs.
Qualifications:
- Active California Physical Therapist (PT) License.
- Graduate of an accredited Physical Therapy program.
- SNF or geriatric rehabilitation experience preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a collaborative team.
